Student Health Center, Klotz

The Klotz Student Health Center (SHC) is dedicated to providing the highest-quality healthcare to CSUN students. SHC services and educational programs are designed to help students maintain and improve their health and wellness, so they can better achieve their academic and professional goals.

All SHC services are provided to currently matriculated students at little or no cost. Basic primary care provided by nurses, physicians and nurse practitioners is free. X-rays, lab tests performed in the SHC laboratory, health education, sexual health exams, nutrition counseling and sports medicine are also offered for free. For a nominal charge, the SHC provides acupuncture, chiropractic, dentistry, specialty gynecology, optometry, physical therapy, and travel and immunization services, as well as a full-service pharmacy for prescription and over-the-counter medications.

Treatment of Minors: CSUN students under the age of 18 must have a signed Consent for Medical Treatment of Minors prior to receipt of services. This form can be obtained at the Klotz Student Health Center or from the website. This requirement does not apply to emancipated minors who can document their status.

After-Hours Nurse Triage: After hours, evenings, weekends and University holidays, a free telephone nurse service (Fonemed), which is contracted by the Klotz Student Health Center, can answer medical questions or direct currently enrolled students to open healthcare resources at their own cost. (This service cannot make or cancel appointments with the Student Health Center, review immunization requirements, clear registration holds or address any nonmedical questions, such as CSUN academic policies and procedures.) The Klotz Student Health Center does not have an emergency room.

CSU Immunization Requirements

As instructed by the CSU Chancellor’s Executive Order 803, entering California State University (CSU) students are required to present proof of the following immunizations to the CSU campus they will be attending.

Students must comply with the requirements no later than the beginning of their second term of enrollment. If requirements are not met, it may result in the inability to enroll in classes.

The CSU requires that all new students are current for the following:

• Measles, Mumps, Rubella (MMR): Two (2) doses or positive titer (blood test)
• Hepatitis B (Hep B): Students age 18 and younger, three (3) doses
• Varicella (Chickenpox): Two (2) doses or positive titer
• Tetanus-Diptheria-Pertussis (Tdap): One (1) dose after age 7
• Meningococcal Disease (A,C,Y,W-135): One (1) dose on/after age 16 for all students age 21 or younger
• Tuberculosis Screening/Risk Assessment (TB)

In addition, the CSU strongly encourages students to obtain the following immunizations, after discussion with their healthcare provider:

• Hepatitis A (Hep A)
• Hepatitis B (Hep B): Students age 19 and older
• Human papillomavirus (HPV)
• Influenza (Flu shot, annually)
• Meningococcal B (Meningitis B)
• Pneumococcal
• Poliovirus (Polio)

For detailed instructions on how to electronically upload immunization documentation to CSUN’s myHealth Portal, see Immunizations. Students will not have access to this portal until they are currently enrolled.

Documentation will be accepted in any of the following forms to fulfill the immunization requirements:

Verification of immunity completed by a healthcare provider, or a childhood immunization record (yellow vaccine card), or high school transcript (if immunizations are documented), or a lab report with proof of immunity from a titer (blood test).

Immunizations and titers are available at the Klotz Student Health Center (at cost). If students qualify for VFC (Vaccines for Children) and/or an HPV vaccine program available at the Klotz Student Health Center, they could qualify for free immunizations.

Student Health Insurance

Students who are not covered by an individual, family, employer or other health insurance plan are strongly advised to purchase health insurance that will cover the costs of services beyond the scope of the Klotz Student Health Center, such as emergency services, hospitalization, maternity care and surgery.

The Klotz Student Health Center is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).
